1. *Assesses the psychosocial status of patients/families/caregivers related to the patient?s terminal illness, environment and caregiver ability to function adequately. The assessment includes but is not limited to, potential for risk of suicide, abuse or neglect and/or pathological grief, bereavement needs and any other special needs related to cultural diversity. Provides an assessment in the patient?s identified residence and assistance when this is not safe and another plan is required. Prepares and submits clinical progress notes which will reflect patient/family issues and goals as it relates to the plan of care. 2. *Carries out social evaluations and plans intervention based on evaluation findings. Counsels patient/family/caregivers as needed in relationship to stress and other identified coping difficulties. 3. Provides information and referral services for patients/families/caregivers regarding practical and environmental needs. 4. Provides information to patients/families/caregivers and community agencies. 5. *Serves as liaison between patients/families/caregivers and community agencies. 6. Maintains collaborative relationships with Hospice personnel to support patient care. 7. Participates in the development and revision of the plan of care and attends IDT conferences. 8. *Assists physician and other team members in understanding significant social and emotional factors related to health problems and death/dying issues. 9. *Participates in discharge planning when appropriate and identifies patient and family needs when the level of care changes. Assists patient/family/caregiver with securing durable power of attorney, evaluating financial issues, long term care placement and funeral arrangements as needed. 10. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practices skills through attendance participation in continuing education and in-service programs. 11. Other duties as delegated by the clinical supervisor. NOTES: Additional Salary Information: Depends on experience.

1. Is a graduate of a Masters program in social work accredited by the Council on Social Work Education and 1 year experience 2. Has a Bachelor Degree in social work. Psychology, sociology or other related field and 1 year experience and supervision by a MSW. 3. Has a Bachelors Degree in social work and employed by hospice before the effective date of the final rule (December 2, 2008). 4. Has clinical experience relevant to the psychosocial needs of patients and families.
